Covered Weight is not a healthcare provider and nothing here is medical advice. This site describes treatments as a category and never recommends one for a reader. Only a licensed clinician who knows your health history can tell you whether a treatment is appropriate for you. Do not start, stop, or change a medication because of anything on this site. If you are having a medical emergency, call 911.
Not a promise of coverage
This site is not an insurance company, an insurance broker, or a navigator, and it cannot tell you what your plan will pay for. What it publishes is what a named, dated, published document says. Coverage rules change, the same insurer sells many different plans, and an employer that pays claims out of its own funds can set rules that differ from any insurer policy or state rule. Your own plan documents control. Call the member services number on your card to confirm what applies to you.
Not legal advice
Pages about appeals describe a process and the bodies that run it. They are not legal advice and they do not create an attorney relationship. Deadlines and procedures in your own notice govern your case.
Accuracy and dates
Every fact carries the date it was read from its source, and the corpus is re-verified each January and July. Documents still change between those dates, and a source can be replaced without notice.
