What this site is
An information service. It publishes what health plan policy documents say, with a citation to each document. It does not provide medical care, pharmacy services, insurance, insurance broking, or legal representation, and using it creates no professional relationship of any kind. The full limits are on the medical and coverage disclaimer, which forms part of these terms.
Accuracy and reliance
We take accuracy seriously and we describe exactly how in the editorial process. Even so, coverage documents change, sources are replaced without notice, and a page can be out of date between one reading and the next. The site is provided as it is, with no warranty that any fact is current for your plan on the day you read it. Decisions about your treatment and your coverage should rest on your own plan documents, your clinician, and the notices you receive, not on this site.
Using the content
You may read, print and share these pages, and quote from them with attribution and a link to the page quoted. Republishing a page in full, or scraping the site to build a competing dataset, is not permitted. The underlying documents this site cites are public and you are free to read them yourself, which is rather the point of citing them.
Trademarks
Product and company names on this site belong to their owners and appear only to identify the products a policy document names. Their use here does not imply any affiliation or endorsement.

This site links to documents and pages it does not control. A link is not an endorsement, and what happens on another site is governed by that site's own terms. Where a link is paid, it is labeled and disclosed, and how we make money explains the arrangement.
